Our dedicated team has worked on Unplanned Extubations (UE) since 2018 and has successfully decreased harm by reducing our UE events by 83%.”

“Our participation in the UE Proactive Safety Huddle Cohort added an additional layer of safety for our patients, this time involving parents to participate in the Proactive Safety Huddle to mitigate a plan to avoid future UEs. WVU Medicine Golisano Children's joined the SPS Central Line-Associated Blood Stream Infections (CLABSI) Reduction Cohort, implementing the tools learned from Proactive Safety Huddles and Walk-Through Talk-Through (WT3), to help educate families on the reduction of CLABSI in high-risk patients.

Our commitment to zero harm has directly improved the patient experience by creating safer, more reliable care processes. Families notice the difference—fewer delays, clearer communication, better coordination, and a team that is proactive rather than reactive.”
