UCSF Benioff Children’s Hospital San Francisco Celebrates Safety Week 2026

SPS is celebrating Patient and Employee Safety Week on March 8-14! In celebration of #SPSsafetyweek2026, we’re highlighting the awesome patient and employee safety improvement work within our organizations! Next up is UCSF Benioff Children’s Hospital San Francisco!

At UCSF Benioff Children’s Hospital San Francisco, patient safety is strengthened through proactive learning, shared accountability, and cross‑disciplinary collaboration. During Safety Week, we’re proud to spotlight teams whose work is advancing medication safety and harm prevention across the organization.

A multidisciplinary group — including Bridget Canty, Lori Fineman, Shannon Fitzpatrick, Theresa Freitas, Kathy Ghomeshi, Deepa Gill, Sarah Lucas, Julie Nguyen, Marina Rawlings, Sylvia Stofella, and Aylin Ulku — has established a monthly forum to adjudicate cross‑bay medication and fluid events using MERP rankings. This work has helped integrate medication safety structures across campuses, align definitions of harm, and support the San Francisco campus in reporting adverse drug events to SPS. The team’s engagement and collaboration have laid a strong foundation for consistent learning and transparency.

In parallel, Rebecca Chigas, Maggie Griffith, Nicole McNeil, and Martina Merz have led proactive harm prevention efforts using the Walk‑Through Talk‑Through (WT3) tool for Hospital-Acquired Pressure Injury (HAPI) and Central Line-Associated Blood Stream Infection (CLABSI) prevention. Through frontline engagement, prioritized actions, and targeted education, early results show zero HAPIs in identified focus areas.

Together, these efforts reflect a strong, system‑wide commitment to safer care through proactive, collaborative improvement.
